Nursing students should develop the ability to continuously evaluate and improve the quality and safety of patient care. This learning activity provides a hands-on approach to practicing quality improvement (QI) processes with real-life errors to expand students' patient safety knowledge. The faculty collaborated with a clinical partner to gather de-identified occurrence reports on various hospital errors from the last few years. These events included laboratory results reported on the wrong patient, incorrect food tray delivery, wrong medication dosage administered, etc. Students worked in groups of 6 to 7 to perform a root cause analysis (RCA) on their assigned real-life hospital error and create the plan portion of the Plan-Do-Study-Act (PDSA) model to improve the processes. To complete the project, students had a QI Class Presentation Day. All groups presented their medical errors through a skit or video recording in the simulation laboratory. They also discussed the RCA and their plan to prevent future errors. Students and faculty discussed the error, RCA findings, and the PDSA model and debriefed after each presentation. At the end of the day, the class voted on a "People's Choice Award" winner for best presentation, RCA, and PDSA. Incorporating a hands-on approach to practicing QI processes through real-life errors exposed the students to the reality of errors that occur with patient care. It captivated their interest in QI processes and expanded their patient safety knowledge.
