Abstract

Managed healthcare, increasing numbers of frail elderly, and complex healthcare delivery systems are but a few of the forces driving the need for nurses to facilitate an effective discharge planning process. Interdisciplinary collaboration is a critical component in achieving desired cost and quality outcomes as patients move along the healthcare continuum. This study was designed to explore the effects of education and experience on knowledge and perceptions of collaborative discharge planning among nurses.
