With my patients, I have seen doctors pay more attention to things like the NT-BNP measurements and other lab tests to decide how important the cardiac output is.

Bob,
There is something called a nomogram. It is a scale. On the left you plot height, on the right is weight. Mark each. In the middle is the BSA or m squared column. You use a square edge to draw a line from left column to the right column passing over BSA. This is the BSA.
