Abstract
Implementing senior leadership Safety Rounds at one home care and hospice organization has strengthened the safety culture and improved communication at all levels. Safety Rounds have led to identification of safety and quality priorities and enabled leaders to act proactively rather than reactively, by identifying safety risks and concerns before an error occurs or a patient is harmed.
