[black small square] In "Don't Forget the Sixth Right" (Letters, July 2009),* a reader said that the sixth right of medication administration is right documentation. When I teach a reentry/refresher course for LPNs and RNs, I stress eight rights. The seventh is right reason, and the eighth is the right to refuse. Nurses need to know why a medication has been prescribed. They must also respect the patient's right to refuse treatment, including medication.
