I'm writing about "'A Ticket to Ride' Protects Patients Off the Unit" (May 2009).* Using a standardized form with critical patient information during transports and off-unit diagnostic testing is a wonderful way to protect patients. I have one additional recommendation: include the patient's isolation status on the form. This helps protect other patients and healthcare workers when patients are on isolation precautions.
We generate our own form for patient transit from our computerized documentation, pulling in all the needed information with just a few keystrokes.
