To determine the "Health Domain" of a Pulmonary Rehabilitation (PR) Program the use of a QI Tool to evaluate the documentation of the team is useful. A PR program that is not regularly evaluating the documentation of their team members can be its demise. Unless an audit occurs from an insurance company with a call for medical records never will the program understand if their documentation is meeting hospital or insurance standards. The INOVA Fairfax Hospital pulmonary rehabilitation program developed a Chart Audit tool that is part of the patient initial rehabilitation chart until discharge. The audit tool does not become part of the patients permanent record. The audit tool allows each team member who is "Charge" for that program to review each patients program chart during the 10 weeks the patient participates in PR and before the patient discharges to determine documentation accuracy and needs. This process of QI will allow the chart at discharge to be complete and accurate. This QI audit tool will insure payment for services provided that are medically necessary.
