The symptoms of achalasia are both physically and emotionally distressing to the patient. Once diagnosed, treatment options include laparoscopic myotomy, pneumatic dilation, and the injection of Botulinum Toxin Type A (Botox(R)) into the lower esophageal sphincter. The purpose of this presentation is to define, specify, and emphasize the vital role of the nurse in assisting with the Botox(R) treatment.

It is known that improper handling of the Botox(R) can denature its composition, and impact the efficacy of the treatment. Correct preparation of the medication and injection technique, including necessary "standard" communication with the physician regarding resistance during the injection (indicating deposition of the medication into the muscle layer), are factors that can potentiate the intended results.

A review of the disease process, pharmacology of Botox(R) with its specific effects on the lower esophageal sphincter, and the step-by-step process of assisting with the esophagogastroduodenoscopy with Botox(R) injection procedure will be presented. The confident, proficient endoscopy nurse, together with the physician, contributes to the optimal outcome of the patient.
