The FDA approved changes made to the safety labeling of itraconazole capsules and injection (Sporanox), which now warns that concurrent use of itraconazole, a CYP 3A4 inhibitor, is contraindicated with the following medications: levomethadyl (Orlaam), cisapride (Propulsid), oral midazolam (Versed), pimozide (Orap), quinidine, dofetilide (Tikosyn), and triazolam.

 

HMG Co-A reductase inhibitors metabolized by CYP 3A4 are also contraindicated for concurrent use with itraconazole.

 

The FDA has received reports of serious cardiovascular events and life-threatening cardiac dysrhythmias in patients using cisapride, pimozide, levomethadyl, or quinidine with itraconazole and/or other CYP 3A4 inhibitors.
