In 1992, Ayello1 developed a mnemonic-A-S-S-E-S-S-M-E-N-T-for pressure ulcer assessment and documentation. The mnemonic has been adapted for use with any type of wound to provide a thorough look at the parameters that complete and enhance a wound assessment. It provides a support structure, in chart form, for clinical decision-making regarding ongoing assessment and reassessment. The mnemonic can be used in any practice setting and according to the guidelines set by the practitioner's facility. This assessment chart can be used daily, weekly, or monthly. It is simple, fast, and can be further adapted to fit individual use.
