New AHA guidelines include extending time to treatment for ischemic stroke. The updated American Heart Association (AHA) guidelines for treating acute ischemic stroke has increased the acceptable time period for mechanical thrombectomy in selected patients from six to 24 hours. The clot-dissolving drug alteplase is recommended for minor stroke patients within three hours. After arrival at the ED, brain imaging should be done within 20 minutes in all patients, but should not delay iv alteplase treatment. Patients at risk for acute ischemic stroke and their families should be instructed to call 911 for an ambulance rather than drive themselves to the ED. This enables emergency medical services to call ahead to alert ED staff to prepare to administer treatment immediately upon the patient's arrival.
