Abstract
The home health Plan of Care (POC) provides the primary means of communication among care providers, is used by regulatory bodies to assess many aspects of compliance, and serves as the road map for care delivery. This article focuses on requirements for the Medicare POC that assure specificity, comprehensiveness, detail, and measurable goals.
