Transitional care reduces hospital readmissions. Researchers compared the use of transitional care-which included a variety of services depending on need, such as a bedside visit before discharge, a comprehensive medication review by a pharmacist, and home visits by a nurse care manager-with standard care in Medicaid patients with multiple chronic conditions. The study, described in the August issue of Health Affairs, tracked 21,375 discharged Medicaid patients; 13,476 received transitional care at 1,325 primary care medical homes across the state. Under the statewide program in North Carolina, for every six patients who received transitional care, one readmission was averted. High-risk patients particularly benefited: 37% receiving transitional care avoided readmission at one year, compared with 25% receiving standard care.
