The letter raises an excellent point-and indeed provides a clear example of how people can easily "read into" the question. If this question asked about a medical surgical nurse, etc, the answer would have been "blood sugar regulation." However, this is a Wound, Ostomy and Continence Nursing Certification Board exam. The primary concern of the WOC nurse is limb preservation. Remember the focus of the exam you are taking, and don not necessarily think about what you would do in your own setting. Focus on WOC content, and the answer is very straightforward.
