Section I.

Section II

 

a. Omeprazole and sodium bicarbonate. Available by prescription-only and over-the-counter (OTC). This brand name is the OTC combination consisting of a proton pump inhibitor (PPI) and an alkalinizing agent. The OTC formulation comes in capsule form, each containing omeprazole 20 mg and sodium bicarbonate 1,100 mg. Indicated for the treatment of frequent heartburn that occurs 2 or more days per week. It should be taken once daily for a maximum of 14 days, 30 to 60 minutes before a meal, ideally before breakfast.

 

b. Dexlansoprazole. Prescription-only PPI; a dual delayed-release formulation. Absorption occurs first in the duodenum and then further down the small bowel. The dosing for symptomatic nonerosive GERD is 30 mg orally once daily for 4 weeks. If symptoms are severe and/or presence of erosive esophagitis, then the initial dose is 60 mg once daily for 8 weeks followed by 30 mg once daily for up to 6 months. Swallowed whole without regard to meals. If unable to swallow, sprinkle granules from capsule onto applesauce. Granules must be swallowed immediately and not chewed.

 

c. Rabeprazole. Prescription-only delayed-release tablet indicated for the treatment of daytime and nighttime heartburn and GERD symptoms. Administered at 20 mg once daily for mild disease and without signs of erosive esophagitis. A patient may start with 10 mg once daily and increase to 20 mg once daily after 4 to 8 weeks. May be discontinued if asymptomatic after 8 weeks. For severe symptoms and/or erosive esophagitis, the dose is 20 mg once daily. If symptoms persist with 20 mg once-daily dosing, the dose may be split to 10 mg twice daily, increased to 20 mg twice daily, or switched to another PPI. Tablets must be swallowed whole and best taken 30 minutes before a meal. Capsules may be opened and contents sprinkled on soft food or liquid. Food or liquid should be at room temperature. A whole dose should be consumed within 15 minutes. The most commonly reported adverse reactions in adults included pain, pharyngitis, flatulence, infection, and constipation.
