The coronavirus 2019 (COVID-19) pandemic has had a major impact on discharging Veterans poststroke to subacute rehabilitation programs. Many of these facilities are not accepting new patients due to fluctuations of in-house COVID-19 cases. Therefore, poststroke Veterans remain as inpatients while waiting for rehabilitation placement. The main efforts of poststroke rehabilitation programs are to improve patients' cognitive, motor, and sensory functions. As the clinical nurse leader (CNL) on a busy telemetry unit, it was necessary to continue caring for Veterans beyond the normal length of stay. This change in status required adjustments in their plans of care. The CNL facilitated a multidisciplinary team, which included the physical therapist, occupational therapist, stroke nurse coordinator, case manager, social worker, and speech therapist, to implement a rehabilitation program to benefit these Veterans in waiting. The team developed individual plans of care similar to subacute rehabilitation programs to make certain the Veterans received and participated in a specialized stroke rehabilitation plan. The focus of the program was to ensure the veterans maintained and/or improved functioning and to prevent a decline in affected areas. Assisting these Veterans to reach their maximize recovery and gain independence for a better quality of life after surviving a stroke was the primary goal of the team despite the pandemic.
