Most publications on nutrition and pressure ulcers (PrUs) focus on interventions for Stage III and IV PrUs. Are there any tips for Stage I PrUs in long-term care?
The nutritional status of patients with Stage I PrUs should not be overlooked. Here are some general nutritional guidelines.
Provide:
* adequate calories: 30 to 35 kcal/kg of body weight
* adequate protein: 1 to 1.2 g/kg of body weight
* adequate fluids: 25 to 35 mL/kg of body weight
If nutritional intake is suboptimal, consider:
* daily multivitamin and mineral supplement
* favorite/comfort and culturally appropriate foods
* assistance with meals
* proper positioning and socialization at meals
* emotional support
* enhanced foods (recipes modified to provide extra nutrition)
* medical nutritional products between meals
* high-calorie supplement with medications.
Remember these points:
* Evaluate renal function before recommending additional protein.
* Each patient has a unique situation; use your clinical judgment to adjust these recommendations.
* Document all interventions in the medical record.
* Request a medical nutrition therapy consultation, if nutrition is a concern.
