This scale measures neurological function and deficits by asking the patient to answer questions and perform physical and mental tests how to use the nursing process to assess the patient, make a plan, intervene, and then evaluate the results of that intervention. We also discuss how to communicate what’s happening to the provider and what interventions should be considered.
