In "Documentation: Name That Vein," a nurse asked why her manager discouraged her from naming the specific vein used to insert an I.V. device. You replied that because many nurses don't know the correct names for all the veins they might cannulate, they may be expected to document only the vein's location (Advice, p.r.n., May 2006). I thought Infusion Nurses Society (INS) standards require documenting the vein's anatomic name. Which is it?
