Beginning January 1, 2006, a patient's medical record must document his language and communication needs, along with his name, sex, address, date of birth, and authorized representative, if any. The new requirement is part of revisions made to the Joint Commission on Accreditation of Healthcare Organizations' (JCAHO's) Information Management standard IM.6.20.
