Joy Ufema pioneered the role of death-and-dying specialist at Harrisburg (Pa.) Hospital in 1973. She's currently clinical specialist at Upper Chesapeake Medical Center in Bel Air, Md., and at Harford Memorial Hospital in Havre de Grace, Md.

One of my patients who's been on dialysis for several years, told me that she wants to “stop all this” but is afraid of dying. She's haunted about what awaits her in the hereafter, yet she refuses to talk about her concerns with a member of the clergy. She seems to feel terribly guilty about something—and I feel guilty leaving her each day in such distress. What can I do to help her?—G.R., ILL.
