Abstract: The Collaborative Health Management Model fosters teamwork between nurse practitioners and physicians based on an egalitarian partnership. It serves to operationalize the "Future of Nursing's" call for Advanced Practice Registered Nurses to deliver high-quality chronic disease management using their full abilities: a unique professional lens, expertise in team-based care, and patient partnerships.
